Complete Application

  1. Begin with birthdate (digits only)
  2. Tab once to First Name, tab twice to Last Name, tab once to area code (enter 3 digits only), tab once to phone number (enter 7 digits only, no dashes)
  3. Tab to fill in MVCC
  4. Tab to fill in Address, City, Zip
  5. Check box for "I am a resident of..."
  6. Tab to Date, then click on Signature line


Signing Digitally

The PDF is embedded with coding that provides for an electronic signature. Follow the steps provided. Below is an example of one stakeholder's experience. Each stakeholder has a different device, operating system, and should expect a slightly different scenario. If the program wants you to create an account for Adobe, go ahead and follow the instructions.

Save the PDF with the same name and in a location with easy access for emailing.

Changing the name may make it unreadable.


Step Five

Take Picture of Photo ID

Use your computer's camera (or phone) to take a picture of your photo ID (e.g., driver's license, ID card, passport) and save the picture in an easily accessible location to email with your application.


MAY YOU SKIP THE NEXT STEP?

If your driver's license or ID card has your current address, you need NO ADDITIONAL documentation. Skip the next step and proceed to emailing your signed application and supporting document(s).


Take Picture of Current Bill

If your driver's license or ID card does not have your current address, you will need to provide another document to prove residency. 

Examples: current utility, cable, or phone bill or other document for proof of residence

The final step is to email the digitally signed application along with the picture of your driver's license or ID card.

IF your driver's license or ID card does NOT have your current address, you will need to email both the picture of your driver's license and the additional document (e.g., utility bill) for proof of residency.
